Insurance option on User profile

We used custom fields that can quickly be updated via an import. This works well, but it would be icing on the cake to:

1. Create an insurance template
2. assign users to an insurance plan bulk or other function (or as purchased then updated by secretaries
3. show on the user view whether the user has current insurance
4. when an incident is created and insurance is chosen as the source of payment for the repair or replacement, the insurance is consumed (unless the insurance can be consumed multiple times for the school year, like what we do)
5. Quickly allows technicians or billing admins to see that a student has used insurance and filed n+x claims and what incidents are related.
6. user profile shows historical insurance participation (extra icing on this cake) so that admins can see prior years of insurance participation as well.
